Implementation/Evaluation Nurse assessed the patients ability to perform activities of daily living and realized where the patient needed assistance. Patient was encouraged to do ADLs on his own, but to recognize and ask for help if he needed it. Patient was able to ambulate on his own the bathroom, perform most activities independently, but required some help from his wife by dischar ge. This goal was met because the patient realized when he needed help and performed all ADLs safely by discharge. f. Expected Outcome 2 Resources are identified that are useful in optimizing the liberty and independence of the patient by discharge from the hospital. xvi. Intervention 1 Nurse will assess what assistance will be needed when the patient is discharged.16. Intervention 2 Nurse will encourage patient to use assistive devices until no longer needed, and reassure patient that use of them can prevent falls and injuries. 17. Patient used assistive devices and help from others when he recognized he could not do them independently.
